Glooko, Inc., a global digital health company focused on helping clinicians address the growing challenges of glycemic safety and diabetes management across the care continuum, today announced new real-world data presented at the Diabetes Technology Society (DTS) meeting demonstrating the use of EndoTool IV for intravenous insulin dosing during labor and delivery (L&D) in pregnant individuals.
The poster, “Use of a Computerized Intravenous Insulin Dosing Application in Pregnancy,” evaluated outcomes among patients requiring intravenous insulin during L&D—a period when tight glycemic management is particularly important and care teams must balance the need to correct hyperglycemia while avoiding hypoglycemia.
The retrospective analysis included 267 individuals across 43 facilities from the 2025 EndoTool IV database and compared individuals with Type 1 diabetes to those with gestational diabetes (GDM) and Type 2 diabetes.
Key findings included:
- No recorded instances of severe hypoglycemia (blood glucose <40 mg/dL) in either patient group.
- Blood glucose <60 mg/dL occurred in just 0.63% of readings among patients with Type 1 diabetes and 0.16% among patients with Type 2 diabetes and GDM.
- Hyperglycemia (blood glucose >180 mg/dL) occurred in less than 10% of readings in either group, with only six total readings exceeding 300 mg/dL.
- These results were achieved across a tight range of 90-120 mg/dL in 80-90% of patients.
“Labor and delivery represents a uniquely dynamic period for glycemic management, when insulin requirements can change quickly and clinicians need to maintain tight glucose targets while minimizing the risk of hypoglycemia,” said Joseph Aloi, M.D., Section Chief, Endocrinology and Metabolism at Advocate Health, Wake Forest University School of Medicine. “These real-world findings are encouraging because they show that personalized intravenous insulin dosing with EndoTool can support tight glycemic control in this population, with no observed instances of severe hypoglycemia across the patients evaluated.”
The findings also reinforce a broader opportunity to improve continuity of diabetes care throughout pregnancy. Glycemic management can span multiple care settings and clinical teams, from outpatient monitoring during pregnancy to inpatient management during labor and delivery and continued follow-up after birth. Creating greater continuity across those transitions is an important part of Glooko’s broader glycemic management strategy.
Glooko recently expanded its ambulatory platform with GestationalCare™, a pregnancy-specific diabetes management module designed to help care teams more efficiently review fasting and post-meal glucose patterns, identify patients who may require additional attention, and support care throughout pregnancy and the postpartum transition. Together with Glooko’s inpatient technologies, including EndoTool IV, the company is building toward a more connected approach to glycemic management across the full care continuum.
“Diabetes management during pregnancy does not begin or end at the hospital door,” said Mark Clements, M.D., Ph.D., Chief Medical and Strategy Officer at Glooko. “A person may need close outpatient monitoring throughout pregnancy, precise insulin management during labor and delivery, and continued support as her care transitions postpartum. These data are particularly encouraging because they demonstrate the potential of EndoTool IV within an important inpatient setting while reinforcing our broader vision: giving clinicians the tools and insights they need to support more consistent glycemic management as people move across care settings.”
“Pregnancy is also a powerful example of why a connected glycemic management platform matters,” Clements continued. “The clinical needs change quickly, but the need for timely data, coordinated decision-making and continuity between care teams remains constant. By expanding our capabilities across both ambulatory and inpatient care, we see an opportunity to help reduce fragmentation and support a more seamless approach to diabetes management during pregnancy and beyond.”
